Impact Team Application

Applicant must be at least 18 years or older and a policy-holding member of Catholic Financial Life prior to application, which means he/she is the primary insured on a life insurance certificate or an annuitant. A minimum of one month’s lead time is required for all Impact Team applications. You must receive approval from Catholic Financial Life prior to completing your project. Upon approval of your project, you will receive a $150 Impact Team funding card and five Impact Team T-shirts. Please have your team members’ names and shirt sizes available before filling out this application.

I agree to complete the approved project while wearing Impact Team shirts and to submit the required Impact Team follow-up report and photos within two weeks of project completion. I understand that submission of both the report and photos is required for the project to be considered complete. I acknowledge that failure to meet these requirements may result in ineligibility for future Impact Team participation. I understand that the team leader must be a policy-holding member of Catholic Financial Life. I also understand that my household is only eligible for up to two Impact Team projects per calendar year. Applicants who complete all requirements will be entered into a quarterly drawing for a $100 gift card. Member benefits and programs are not guaranteed contractual benefits. The interpretation of the provisions of these benefits and programs is at the sole discretion of Catholic Financial Life.
