What is needed to file a claim/report a death?

The following information you should have ready at time of notice is:

  • Insured's name
  • Date of birth
  • Date of death
  • Contact person's name, address and telephone number
  • Certificate numbers, if available

To file a claim/report a death, please complete and return the claimant statement we will mail to you along with any additional forms that may be required dependent on the claim.
